Group Volunteer Waiver

Known Risks of Harm Associated With Indoor And Outdoor Volunteering/Educational Programming at Various Locations


1. Animal
*Scratches
*Bites
*Venomous snakes

2. Plant
*Poisonous plants
*Plants that cause allergic reactions
*Plants bearing thorns

3. Insect
*Stings and bites - mosquitoes, bees, wasps,
flies, ticks
------- Allergic reactions
------- Disease or parasite carriers (i.e. mosquitoes, ticks)

4. Climate/Weather
*Extreme heat/cold
*Dehydration
*Heat exhaustion

5. Environmental
*Slips, trips, and falls due to uneven surfaces, walkways, or trails

6. Tool Use
*Blows and cuts to the hands or other parts of the body.
*Eye injuries due to the projection of fragments or particles.
*Sprains due to very abrupt movements or strains.
*Electric shock.
*Crushing injuries and amputations.
*Repetitive-use injuries.
*Hearing loss.

THIS LIST OF KNOWN DANGERS IS NON-COMPREHENSIVE.

It is acknowledged by the individual completing this document (participant) and Houston Humane Society Wildlife Center (sponsor) that certain risks of harm do exist within the scope of the volunteer opportunity over which the sponsor has no control. A noncomprehensive list of risks associated with such activities will be posted at the event and included in digital copies of this document for the participant’s knowledge.

The participant agrees to hold Houston Humane Society Wildlife Center harmless for any and all claims for damages, including personal injury or death, medical expenses, disability, lost wages, and property damages which may be incurred by the participant while engaged in this volunteer/educational/outreach program during the period of the Volunteering Event. I hereby agree to waive any and all claims, demands, damages, actions, or suits of whatsoever kind or nature by Houston Humane Society Wildlife Center and/or their staff.

I authorize Houston Humane Society Wildlife Center personnel to take such action as seems appropriate to protect the health and physical well being of myself. I also give my permission for any emergency medical care or treatment by a physician, surgeon, hospital, or medical care facility that may be required, and accept responsibility for the cost.

I understand that the Houston Humane Society Wildlife Center is under no obligation to provide, carry or maintain health, medical, travel, disability or other insurance coverage for any Volunteer. Each Volunteer is expected and encouraged to obtain their own coverage. If participant has health insurance, participant understands participant's personal health insurance is participant's primary coverage.

Participant agrees that in the course of participant's participation in the event, participant may have access to sensitive or proprietary information. Participant agrees to maintain the confidentiality of such information, to use such information only as necessary to do my job as a volunteer, and to comply with Houston Humane Society Wildlife Center for applicable policies regarding such information.

Pictures may be taken during any/all activities hosted by Houston Humane Society Wildlife Center. By acknowledging this waiver, participant agrees to pictures of participant to be used for organizational literature.

By completing this waiver, I affirm that the facts set forth in it are true and complete.
